Introduction: Pleural effusion is the accumulation of fluid between the parietal and visceral pleura (pleural cavity). This may be due to infection, malignancy or inflammation of the parenchymal tissue, or congestive heart failure. This case is interesting to discuss to increase public awareness and serve as a guideline for preventing this condition.

Method:The method in this final scientific work uses a case study method with a nursing care approach to Mrs. I with respiratory system disorders: pleural effusion in the St. Maria inpatient ward, Santa Elisabeth Hospital, Medan..
Results:The signs and symptoms of pleural effusion were all consistent with theory and field data. Furthermore, the nursing diagnosis was also based on the SDKI. Interventions for clients with pleural effusion included nebulizer therapy, semi-Fowler's positioning, pursed lip breathing, and water seal drainage.
Conclusion:Therefore, it can be concluded that the diagnosis of ineffective breathing patterns was partially resolved because a pigtail was installed, acute pain was resolved on the second day, nutritional deficits were not resolved, activity intolerance was not resolved, the risk of unstable blood glucose levels was partially resolved, sleep pattern disorders were not resolved and skin integrity disorders were not resolved because intervention and implementation were not optimal due to time constraints.
